Our Mission

The mission of Destiny School Health Services Department is to provide high quality health care that enables students to optimize performance physically, mentally, and emotionally to achieve academic success while minimizing interruptions in the classroom.

  • Providing effective health education that motivates students to develop and maintain healthy lifestyles
  • Administering health screenings to identify and address deficits that impact academic success
  • Identification of community resources to provide opportunities for health care access for families and staff
  • Monitor, advise, and respond to trends in local and worldwide health issues that may affect the community

Immunization Requirements

To protect all children against serious vaccine-preventable diseases, Arizona school immunization laws require students to receive immunizations before entry to school. The laws and rules governing school immunization requirements are Arizona Revised Statutes §15-871-874; and Arizona Administrative Code, R9-6-701–708.

Diphtheria, Tetanus, Pertussis (DTaP or DT or DTP)
Polio (IPV)
Haemophilus influenzae (Hib — Preschool students)
Hepatitis A (Hep A — Preschool students)
Measles, Mumps, Rubella (MMR)
Hepatitis B (Hep B)
Tetanus and Diphtheria Booster or Tdap (Required for students 11 years and older)
Varicella (Chicken Pox)
Meningococcal (Meningitis) — for students age 11 and older

Immunization Course

If you are comfortable with your understanding of vaccine preventable diseases, you may choose to complete an exemption form. Please click the links below to obtain the appropriate exemption form for students in Kindergarten through 8th grade.

Hearing & Vision Screenings

Hearing and Vision screenings are performed at the beginning of the school year in conjunction with Arizona State Law and Department of Health Guidelines. Screenings are performed each year for: Preschool, Kindergarten, 1st, 3rd, 5th, 7th grades; students new to the school; retained students; and special needs students.

Medications at School

Medications are administered by the health staff according to Destiny School policy. All Prescription Medications, including inhalers, must be in a pharmacy box/bottle with prescription label attached with the child's name, medication name, dosage and time of day clearly stated.

Parents/guardians may enter the JupiterEd Parent Portal and complete the Annual Update giving written permission to the staff to administer over the counter medications, including acetaminophen and ibuprofen. This must be completed each school year.
